Bush Administration Saw the Market as Key to Health Reform
by Doug O'BrienJudging by their unprecedented use of the word “unprecedented” to describe everything the Obama administration has done it appears that they truly think they have fundamentally changed the national landscape in one short year.
Of course, those outside the delusional bubble of the White House know that the only truly unprecedented thing this administration has done is to destroy its own popularity faster than any other modern president, primarily thanks to its ham-handed push for a left-of-center realignment of the nation.
Nowhere is this more apparent than in the titanic health care “reform” struggle. The administration cannot seem to grasp the reality or the reasons for the public’s rejection of Obamacare. The White House is correct that Americans want to reform health care and make it more affordable, accessible and understandable. But the people know intuitively that a government takeover, or just a much bigger government role, won’t achieve those goals.
Reforms that would result in improved national health, better access, and enhanced cost effectiveness were well underway under the Bush administration. The kinds of reforms that many pundits have suggested could be achieved if the White House would ditch its overly ambitious plans and start from scratch.
When it comes to some of the most popular elements of “reform,” the Obama team has simply picked up Bush initiatives and represented them as their own ideas.
The Bush administration launched a major initiative to promote use of new health information technology. The Department of Health and Human Services, under two very capable secretaries, Tommy Thompson and Mike Leavitt, launched complex efforts to create interoperable health IT standards, to promote the adoptions of electronic medical records, the aggregation and exchange of data throughout the system, and the use of this data to improve outcomes. The Bush administration also knew that the only way to get a handle on how and why costs were increasing so rapidly and what to do about it, was to have consistent and universal data to examine.
Secretary Mike Leavitt traveled the country tirelessly to promote the creation of health information exchanges—in which health care providers, in cooperation with employers and public health agencies built infrastructures and designed systems to begin the collection of this vital information.
But the difference between the Bush and Obama administrations was that the Obama team sees this data aggregation as the key to centralized control of health care, while the Bush team saw it as the foundation of a value-driven health care system where consumers were informed and empowered in making critical health care choices for themselves.
The Bush administration also launched an aggressive expansion of Federally Qualified Health Centers (FQHC’s) to expand access to traditionally underserved communities, particularly in minority and rural areas. During the Bush tenure, visits to FQHC’s rose over fifty percent, providing easier access to community-based primary care services—the lynchpin to improved overall health in the economically disadvantaged population. These clinics are vital to providing patients with the “medical home” the Obama team often touts, which was the same objective in the Bush administration.
The Obama administration also talks about overall population health, or “wellness,” as if they had invented the concept. They also crow about how they will harness “prevention” to lower health care costs. These ideas were well ingrained in the Bush HHS policies. But the Bush team knew that the government couldn’t wave a magic wand and change behaviors that hold the key to improved health.
Under Bush two new programs came online that offer us a realistic blueprint of how real reform can work and how the government can be a helpful partner in empowering the patient/consumer, better manage cost, and enhance the quality of care. They are also a strong counterpoint to the Obama approach.
The Medicare Advantage program provided beneficiaries the option of enrolling in a comprehensive insurance plan that would end the patchwork of Medicare Part A, Part B, Part D, and private insurance. Millions of Medicare recipients spend billions each year to purchase private insurance to cover the gaping holes in government coverage. Medicare Advantage allowed seniors to get coverage from one private insurer in a managed care construct that better controls costs and delivers better service to the patient.
Medicare paid a higher premium for Advantage plans, which allowed higher payments to providers to encourage participation in the program. Satisfaction rates for Medicare Advantage are sky high and early indications are that they enhance the coordination of and effectiveness of care for beneficiaries.
Democrats want to gut Medicare Advantage because it features private insurance participation. The government’s role is to set standards and subsidize coverage and then get out of the way. Insurers design products to appeal to certain customers and then market them accordingly. Patients maintain choice and can vote with their feet if they don’t like the quality or service they are provided.
The Bush administration also supported and implemented the Medicare prescription drug program. While many conservatives thought it was a costly expansion of a government entitlement (and it was), it was at the same time a sensible add on and maintained the role of private insurance and consumer choice.
Again, the government subsidizes the purchase of drug coverage on a sliding scale according to need. The individual picks an insurance plan according to their wants and needs. Insurers design plans with customers in mind and are financially rewarded by building a better mousetrap. The program overall has benefits on the wellness and prevention side. Since the taxpayer is already on the hook for seniors’ health care, it makes sense to pay for the drugs to manage hypertension rather than waiting for the very costly heart attack or bypass surgery.
The Bush administration also promoted Health Savings Accounts (HSA’s) that allowed individuals to save tax free money to pay for routine health care costs out of pocket while purchasing a low-cost, high-deductible insurance policy for unexpected major health issues. HSA’s have proven very popular for both employers and beneficiaries. For families with routine health issues, they make sound economic sense and also encourage patients to look for cost-effectiveness in purchasing primary care.
In comparison to the proposed expansion of government direct involvement and control over health care, Medicare Advantage, Medicare Part D and HSA’s, offer examples of how the government can help individuals according to need, but allow them to maintain their control over their coverage and care. Because they all represent counter-arguments against to the argument that only a government takeover of health care will work, Democrats hate these programs or want to either eliminate them or make government the dominant player.
The Bush administration was forward looking in its promotion of prevention and wellness programs. It was a pioneer in harnessing information technology to improve the quality of care. The Obama administration simply continued the push for these ideas that were well accepted across the ideological spectrum.
What the Bush team, and particularly Secretary Leavitt did that was the direct opposite of the Obama White House, was try to bring the idea of value back into the health care sector. They knew an empowered and informed consumer would make cost-effective choices for themselves and their families and that the resulting competition and improved quality would lead to more manageable costs and better overall health.

This is not even close to being over. The present health care cost trajectory is unsustainable and needs to be addressed in the congress. Democrats need to scrap this bill and offer up legislation that is easy for voters to grasp and let's have some votes.
Bush gave a HUGE amount of money to the Pharmaceutical Industry though his Senior Drug benefit expansion. That was not market based at all. And it isn't private insurance when it is tax dollars that are being spent no matter what sort of quasi-market figleaf you want to use. By that reckoning, Obamacare mandating we buy health insurance is "market based."
The problem I have with both parties is that they are trying to solve all of our problems at the federal level. Politicians hear the call of "This is broken. Please help us, fix it." And immediately there is talk of the federal government stepping in to solve it. Our Founding Fathers purposely decided to give the Federal government very little authority over social issues because they gave them the power to raise, print, and manage currency. The states should be focused on the social issues, not the national government.
We have ignored this at our own peril– looking for an easy national solution has given authority to a body who can simply print more money to make anything happen…but the one thing they can't do is be effective with these dollars because the problems are simply too large and complex at the national level. If we just followed the principles of the Constitution, we wouldn't find ourselves in these messes.

Vote all the lawyers out of Congress and then pass Tort reform. Pass legislation that insurance companies can sell health insurance across state lines. There is no good reason that this shouldn't be done.
If and when tort reform, medical plan cross border availability, and remedies for Medicare fraud are in place, I'll consider more tweaks to health care reform. Reform can only be 'built' a block at a time and each 'block' (bill) can not exceed the length of the Constitution and be absent of legalese.

Actually Market based would be be more FSA and HSA accounts which the democrats actually seek to reduce in their proposals. If consumers of health care could save and shop for health care with their own money you would see a transformation in the cost. Consumers only look at one thing right now the EOB co pay amount. IF we were paying for it with an HSA we would be asking for itemized bills and arguing over 15 dollar aspirins.
From my clinical psychology background, the narcissistic Obama will have tremendous anxiety wrapping his head around what happened in Blue state Mass yesterday.
HSA is pretty good, but ever notice they try to put some ridiculous poison pill in it that you have to use the money or lose it. How stupid, but the intent is to prevent people from using them.

FSA and HSA accounts
Well a person has to have money to put in these accounts right? I suppose my family could stop eating or live in the street. The problem is companies switch you over from a co pay insurance plan to an HSA and contribute nothing to the plan and you are making the same take home pay. Don't say this is not true. It happened to me personally. So what can be done so a situation like this does not happen? All the company did was shift the cost to it's workers and provided 0 assistance.
Yup. Exactly.. Plus with many states trying different solutions to the same problem the best one would be found and implemented.

This sentence strikes at the heart of the Obama agenda. They don't believe in empowered and informed citizens, rather, they believe Government should be empowered to make all decisions for the individual.

I think we can agree on some points. Tort reform will reduce many costs because malpractice insurance premiums will go down. Another thing we need to do is make the loser always pay when a case is settled no matter how long it takes to settle. This will always stop frivolous cases from even being filed in the first place. I, even as a conservative, would be in favor of all doctors being licensed nationally so butchers can’t flee to another state to harm someone else. Also, something I saw was prices where I lived were high for medical care. Then, a “Reddie Clinic” was set up in the local Wal-mart and within a month prices dropped by 30%. People who have pre-existing conditions through no fault of their own, should not be discriminated against. The rest, should be willing to “adjust” their premiums accordingly. If they were insured, and lost insurance through layoffs, or cancellation, should not be denied coverage when they are able to pay for insurance again. And finally, we the people should keep Medicare and Medicaid available for those citizens who cannot get any insurance.

I pay my own taxes for fed, property, health care, state etc. The level of anger and contempt for the mismangement and gouging goes way up when it's not hidden behind the 'Take Home' curtain and you have to cut a check every quarter.
Of course they know this don't they, if the average american had to start budgeting for all of that I suspect the economic landscape would look quite different.

Walker, unfortunately, the ratification of the 17th Amendment trumped the "States Rights" 10th Amendment. Until the 17th Amendment is repealed, providing the States with direct election of our Senators by the State Legislatures, resulting in the States having a voice to poptect the vested interests of each State, I'm afraid it's a moot point, my friend. Hanzo, I'm not sure of your interpretation of the 17th Amendment. No question it affected the process of how we get our Senators today, but I don't read it quite as liberally as you are in relationship to the 10th.
If you have legal precedent that sheds more light, I'd love to see it, but I do not read anything into the 17th Amendment that would suggest that we can't and shouldn't restrict the powers of the federal government. Clearly our courts and federal representatives have pushed this envelop and our state representatives have allowed this to happen, but I don't see how the Constitution now makes it impossible for us to change without a repeal of the 17th.

By changing the method of election to popular vote, going from election by the individual States Legislatures to the pop vote, the feds more or less eliminated States Representation of said States vested interests.
This(17th Amend.) is a prime example of the Federal Government intruding on States Rights, possibly the best example to be seen.Once again, in one fell swoop, States Rights were abrogated to an extent that hasn't been equalled since………

…… Now, when the States have a matter to settle with the Feds, they must seek remedy through the courts(having no reresentation in the Fed Legislature). This is in evidence by State AG's suing the Feds over healthcare. If the 10th were followed to the intent and Letter of the Law, healthcare would be a moot point, as would much(most) other legislation, as it's unconstitutional in the respect that the backroom "sweetheart" deals give one State advantage over another(earmarks anyone?).
How does this happen? It happens because in this day and age of special interest groups and PAC's, Senators are no more than glorified carpetbaggers, not being loyal to the vested interests of the State they "represent", but to their constituents(or financial backers). I hope this clears up things a bit for you on the question of eroded States Rights and how the Feds have gained yet another foothold over our everyday personal lives. Thoughts? The reason the Founding Fathers, in their infinite wisdom, gave the States broad and sweeping powers(usurped by the Feds today), was so the States could, in and of themselves, keep the Federal Government in check, as is, I believe, the point of your post above. The Civil War took care of the nasty little question of States Rights for the Federal Government, the 17th Amendment sealed the deal, so to speak.We are not supposed to have to reign in the powers of the Federal Government, the Union of States is given this task to accomplish for us. The Constitution(as I'm sure you are aware)is NOT a fluid document, open to interpretation, as so many pols and, most unfortunately, the courts(all the way to the Supreme Court)would have us believe…………. Hanzo, I believe I see your point. The 17th amendment IN EFFECT takes the power away from the states and gives it to the federal government. Now, the only recourse is the courts, which is that other constitutional check and balance that often does not work as well as it should.
I'd still like to see a state force the issue some time and make the federal government demonstrate how they are acting constitutionally. Because, as written, the 17th does not supersede the 10th, it simply causes to be irrelevant (to your point). And the states traditionally haven't put up a fight for sovereignty because they keep getting the opportunity to take a piece of candy out of the fed's magic candy jar, where budgets never need be balanced. At the end of the day, all of these issues end up in the courts anyway, which is how we got the 17th amendment to begin with.

Excellent observations Walker. To expound just a tad on one comment you made, States also don't put up a fight against the Federal Government because they have no RIGHTS. Since the 17th arrogated the States Right to elect their(individual States) Senators as their(States) representatives in the Federal Legislatures, the States therefore have no such Rights. No Representation, no Rights. The States then have almost no choice but to revert to fallback position to "take a piece of candy", as you said. I guess we get the government we deserve, but most people were kept in the dark as to what the true intentions, or end result(being charitable here) were going to be when crafting these insidious pieces of treachery.
